FY26 Income Verification Form

The state has updated Income Verification requirement and the EdChoice Income Verification system will only accept 1040 Federal or State income tax returns. or approved affidavits (for Expansion only) for income verification. If W-2s statements, paystubs, and other income documents are submitted, the application will be placed in Correction Needed status. Please use this link to access the new form.

In summer 2023, the Ohio state legislature expanded the EdChoice scholarship program to include all families wishing to attend a private school. The scholarship amounts are determined by household income, and can be up to $8408 per student, per year. We encourage all our families to apply for these funds and are happy to help with the process.

To apply for a Traditional or Expansion EdChoice scholarship, please complete the following steps:

  1. Download the Request Form and fill it out in its entirety. This form should be signed by the primary parent entered on the form. Please note that we require “wet” signatures on all forms to ensure that we are meeting the state’s changing requirements.
  2. Provide an acceptable Proof of Address. This can be any utility bill (electric, gas, water, sewer, cable/internet) that shows a matching service and mailing address in the name of the primary parent/guardian.
  3. Provide a copy of your student’s Birth Certificate.
    • *The Request Form, Proof of Address and Birth Certificate should be submitted to Alter High School. These documents can be brought to the main office during regular school hours or can be submitted electronically to alter.edchoice@alterhs.org. *
  4. New Expansion applicants, as well as Traditional applicants who wish to qualify for low-income status, need to complete the entire Income Verification process to be awarded a scholarship.
    • *This process is completed directly with the state of Ohio and should commence AFTER we confirm that we have entered your student’s application in the state system.*

To renew your existing EdChoice scholarship, each family must submit an EdChoice Renewal to the school along with supporting documentation. A complete renewal includes:

  1. Renewal Form
  2. Proof of Address
  3. Income Verification is optional, and would, again, be submitted directly to the Ohio Department of Education.

2025-26 EdChoice Information

If you have not yet submitted your student’s EdChoice Renewal/Application for the 2025-26 school year, please do so as soon as possible. FACTS agreements will be credited with Provisional EdChoice Scholarship amounts for only those scholarships in an eligible status.

For current EdChoice Traditional & Expansion recipients:

  • Please complete the Renewal Form and provide an acceptable proof of address document that is less than 90 days old.
  • The form should be completed and signed by the primary parent of this year’s scholarship. “Wet”/handwritten signatures are required on all forms to ensure that we meet the state’s changing requirements.
  • Expansion recipients do not need to have their income reverified to renew the scholarship, however, to qualify for low-income status, you must have your income verified each year you desire this status.
  • Renewal documents for 8th grade students attending Alter in 2025-26 should be returned to Alter High School for processing.

For new EdChoice Traditional & Expansion applicants:

  • Please complete the Request Form and provide an acceptable proof of address document that is less than 90 days old and a copy of your student’s birth certificate.

For all renewals and applicants:

  • Please be sure that all information entered on renewal/request forms matches the birth certificate and proof of address documents.
  • Documents can be brought to Alter’s secure lockbox at the front of the school or emailed to alter.edchoice@alterhs.org. Please email each document as a separate PDF in one email.
